Please print out and complete this Health History Form (PDF file) and bring it with you prior to your first appointment or you can fill out the form online while making appointment.
If you were involved in an accident, please also complete this ICBC Injury Claim Form for Massage Therapy.
Blue Cross does cover those who are eligible. A doctor’s note IS required. Claims need to be sent to Blue Cross or the RCMP Occupational Health Services Office and must be received within 18 months of the date of service or we can submit your claim on your behalf.
Extended Health Insurance
Most extended health insurance plans cover Massage Therapy. Please check with your individual health insurance provider for details of your coverage as some providers require doctor’s notes for massage service. We also bill directly to all major extended health care companies e.g Blue Cross, Telus Health, Green Shield, Manulife, Great West Life, Sun life.
ICBC covers $23.00 cost of the Massage therapy treatment, up to 12 treatments after the date of accident. No doctor’s note is required. You will be required to bring in your claim number, adjuster’s name and contact information for your first appointment. We will direct bill ICBC for a portion of your treatment cost.
MSP Insurance (Carecard)
Patients on premium assistance can claim $23 of the cost of their massage therapy treatment through their British Columbia Medical Services Plan (MSP). Patients are entitled to 10 total visits per calendar year and will be responsible to the remaining cost of their visit. We will direct bill MSP for a portion of your treatment cost.
WorksafeBC, formerly WCB
WorksafeBC covers injured workers for their visits and a referral from a physician must be received before a Registered Massage Therapist can treat an Injured Worker. You also are required to bring in your claim number for your first appointment. A confirmation prior to your first treatment indicating that your claim has been accepted is essential.
